Guidelines and Program Requirements Requirements WebRefractive Surgery Packet Submission Guidelines: Refractive Surgery packets will be accepted every 1st and last Thursday of the month from 12:45 p.m. to 3:00 p.m effective November 2024. Only packets submitted by the refractive surgery applicant will be accepted. Packets cannot be delivered by anyone else except the applicant.
